If renaming the simple cgi to simple.cgi can solve this problem, make
sure your setting in your catalog.cfg is right. E.g. CGIURL  
/simple/simple.cgi

I have had the same trouble/problem with WIN NT and renaming to
simple.exe had solved my problem. I hope, it helps you.
