I set the alias just as "/simple" during makecat.. I originally thought
this was why I was getting the "Undefined catalog: /simple.cgi/index" error
so I installed another catalog without the alias... same problems.... 

I am 99.9% sure this is not a MV problem, but a RAQ2/cgiwrap problem. Have
you installed MV on a RAQ2?
