It is hard to figure out without looking at the sources. You have to
replace __MENUBAR__ by [include pages/menubar] etc. _and_ set for any
language you are using in the catalog configuration:

Locale de_DE ReadFile 1
Locale fr_FR ReadFile 1
Locale en_US ReadFile 1

Remember to restart MiniVend/reconfigure the catalog after modifying
locale.asc. BTW: You can use MiniMate to edit the locales database.
