Perhaps it depends on where the file is that you wish to include.  In these examples, MiniVend's checkout module is including from the pages directory, which is the parent of the ord directory in which checkout.html is contained.  Correspondingly, logobar, etc., is in the pages directory, which is, of course contained within the particular catalog directory with which you are dealing:
 
checkout.html:[include pages/logobar]
checkout.html:[include pages/menubar]
checkout.html:[include pages/copyright]
