Fax option doesn't work on checkout

I have minivend 3.12beta6 with CyberCash 3.2 installed.  It seems to 
be validating the 4111 1111 1111 1111 card number fine and all works
well.  However, if the user clicks on the "I will call with..." it 
fails with a CyberCash error.(No card number in form.)

If anyone has an answer to how to solve this I would appreciate a 
response.  I know there are others who have the same issue and would
like to see the response in the list.
