[mv] Converting 3.14 Checkouts to IC

4 hours have now passed. There are 3 more steps to get the next 
online store I am doing online. Taken from pre 3.14 to current.

I've scowered the archives but no luck.

The checkout just needs a checkbox if the user wants to callin the 
credit card information. if it's not checked then credit card info is 
expected.

NOW this might not be working cause I haven't added the PGP info yet, 
but I think it should still display the CC info.

With all the variations of code, If I am accepting Call-in 
information it works. when I put in credit card info and hit submit 
the report/receipt show.
   Payment Method: Credit Card (default)
