$.02 tip:

Try setting mv_cyber_mode to minivend_test and see how that goes,
( * I think that will work w/ authorizenet)
it should accept the order (first time through) and post some stuff to
the error_log. If it does not then you know the problem is somewhere
in your config and/or pages.

If it does go through, then perhaps there is another problem specific to
authorizenet but still could be a config/pages problem.

I don't remember if [data session cybercash_error] is available or not
when using authorizenet as noted below but you could try putting
it on the failed page for more feedback.

*- Depending on how you integrated minivend with authorizenet.
(did you use CCLib.pm with Net::SSLeay or your own way?)

Only other note: you say it gets posted to authorizenet, but is
that before you do the refresh to get the receipt page or after?
