I used the form_mail code that comes with MV.  It shows up in the top
directory.  I modified it to generate the From:, Reply-to: etc. lines
correctly.  Only if an end user looks at every single header sent back by
the mailer can they determine that something called minivend was involved.
Many mailers won't even let them see that much information.

I call the form_mail code from receipt.html, but you could probably call
it from report as well...  I have some other issues with e-mail that
aren't solved by this.
