This is almost always a user pressing the "stop" button. If they do that
after button press, or on input before the form data is fully transferred,
it can easily happen.
The other scenario is pressing "BACK" with a very small cache.
It is actually a good thing because taking a transaction without complete
form data is not so good.
The best way to reduce the number of these errors is to improve your
system performance so people infrequently press "STOP".
