I originally had problems with mine because I didn't keep the country.asc,
shipping.asc, and the references in the HTML correct so it wasn't linking properly at
the country level and always returned a zero freight.

I learned about this by spotting errors in the catalogs error.log file and then traced
it back to the checkout.html page...
