"Place order" returns you to the checkout page

I've been going in circles with this for days. The order form will be
working fine. I'll try it again in a couple hours and everything will work
fine until I get to the checkout page. The checkout page will no longer
have the "No" selected in the "Send an email copy of my receipt" comment.
When that happens, "Place order" will no longer submit the order. It just
keeps returning to the checkout page. I've restarted the Minivend server,
reconfigured, etc., with no luck. Any suggestions?
