State based handling charge

I would like to add a state based handling charge on the Checkout, Receipt
and Report pages. I want it to appear as a separate line on these pages.
The list of states is fixed. MV3.12 on unix.
I have read the docs a number of times, but the answer has not revealed
itself to me. Any ideas on how to do this. Thanks.
