Hi Michael,
Check the documentation - there is information on heavy items which could
probably be easily adapted. See reference-shipping-handling
(http://www.minivend.com/minivend/docindex/15.07.Handling_charges.html).
I personally would make a note on the order form saying "COD Shipments will
have $5 per box added." and let the checkout process complete with the
non-COD amount, since the customer isn't paying by credit card anyhow.
