[ic] Shipping Rates based on Simple Price Table?

I have a customer that wants a shipping table like this:

Up to $50.00 =    $  6.95 
$50.01 to $100.00 =    $  7.95 
$100.01 to $150.00 =    $  8.95 
$150.01 to $250.00 =   $ 10.95 
$250.01 to $500.00 =   $ 14.95 
$500.01 to $600.00 =   $ 18.95 
$600.01 and up =   $ 22.95 


No separate UPS/FedEx charges.  Just this simple table.   How can I 
implement this for them?
