As you have identified, you will need to create a new column in the
orderline table to store the per-item comments.  The transaction
table, defined with the Foundation demo, includes a "comments" column
which you may use to store your per-order comments.

I don't see that you'd have to make any changes to the inventory to
implement your idea.
