I would guess what is happening is that your catalog is trying to use the 
[ups-query] tags to get the shipping totals for each of those UPS shipping 
options. That will only work if you have the Perl mod Business::UPS and any 
dependencies installed, and the computer running IC is connected to the 
internet and is otherwise unencumbered in accessing UPS's servers. If 
either of those are not the case, then that would be the problem.
