Change </FROM> to </FORM>.

Change </loop> to [/loop].

Change <OPTION VALUE="[yaproductsdb-code] [selected...
to     <OPTION VALUE="[yaproductsdb-code]" [selected...
(note the missing end-quote.)

There's bound to be more.  Give the code a general clean-up and
then look for typos and things like that.
