[ic] IC 4.9.5 Shipping Method Empty

Hi,

I just installed a fresh installation of IC 4.9.5 and everything runs
perfectly except for shipping, whereas the shipping method box is empty
on the checkout page and there is an error message:  "Note: 1DA: Missing
ConsigneeCountry".

I have set the following shipping values:
  SHIP_DEFAULT_COUNTRY US Shipping 
  SHIP_DEFAULT_MODE PERD Shipping 
  SHIP_FLAT_DOM_RATE 0 Shipping 
  SHIP_FLAT_INTL_RATE 0 Shipping 
  UPS_COUNTRY_FIELD US Shipping 
  UPS_ORIGIN 91403 Shipping 
  UPS_POSTCODE_FIELD zip Shipping

I haven't changed anything on the shipping tables.

I checked the archives and the ups tag "fix" has already been
incorporated into 4.9.5 so that is not the problem.  Any ideas?
