[ic] Item add problem after upgrade 4.8 to 4.9

Subject: [ic] Item add problem after upgrade 4.8 to 4.9

There seems to be a problem with adding an item after this upgrade. You
can do it by directly editing the table but the "add item" does not
work. Is there any documentation for this upgrade I can reference or has
anyone dealt with this alread?
