This sounds familiar.  The special pages directory may not be set 
correctly.  look for user_violation.html in 
CATROOT/yourcat/special_pages.  If it is there, you need to modify the 
SpecialPage directive in your catalog.cfg to point the correct path: 
../special_pages/user_violation.html
