You have no key "username", as it says.

Try adding:

	<INPUT TYPE=hidden NAME=username VALUE="[data session username]">

If anyone can come up with a better error message, please send it to me.
Perhaps:

    Key variable 'username' appears to be empty, won't set blank key.
