[ic] Gift certificate page generator?

I'm trying to locate the template that controls how the gift certificate 
page (accessed from the gift certificate link on the left hand menu in the 
foundation store) is generated. Currently, a bug exists where the form is 
closed before the submit button is output, making it non functional. This 
is in the latest 4.7.x CVS.
