[ic] Admin Page Errors (ship_edit)

When using the Admin interface for one of our catalogs I recieve the
following error message when attempting to 'Create New Method' on the
Shipping page:

Sorry, the page (admin/ship_edit ui_shipmode=new ui_header=Create method)
was not found 
The requested item (admin/ship_edit ui_shipmode=new ui_header=Create method)
was not found. You can return to browsing our catalog , if you wish. 

The order/checkout functions of the catalog are working fine.  I also tried
creating a new, test catalog from scratch using makecat, and it behaves the
same way.  I re-installed all of the recommended perl modules to make sure
they were working properly, and they are all installed.  

I am also experiencing another problem, I don't know if it is related or
not.  When going to the Tables page, the listing of Database tables is as
follows:

ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR
ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR ERROR
ERROR 

Any thoughts?
