[ic] Shipping Refresh

Hi dudes and dudettes,

Have been reading a lot from the mailing list but not contributing. I
find that when you send an item to the checkout, the shipping rates
are static, regardless of whether you take air or surf.

Here's a short fix for it... (Just add the onChange part in
pages/ord/checkout.html)

<SELECT onChange="this.form.submit()" NAME=mv_shipmode>

Also, do any of you find that when you make a purchase, and checkout,
you have to change the country (refresh) to actually view the proper shipping
rates? Is this a bug that has been addressed?
