[ic] Text Field in Order Form

Hello,

Is there anyway to place a text field in the order form that will allow a
user to enter text about an item, then pick up that information on the email
to the vendor.

One example of this might be ordering a T-shirt with a name on it.  The user
should type the name they want on the shirt, then the vender can fulfill the
order.

I am aware you can do this with select boxes and checkboxes, but unsure
about a text box.

I saw in the archives this was not possible in 1999, but thought it might
have been added since.
