I cannot answer all your questions, but I may be able to give you some
infos that may help you.

Cybercash is configured in the catalog.cfg with the Directive
"CyberCash   yes" and the setting of some variables like CyberHost and
the like. 
CyberCash is tested in minivend (the main program daemon) on line 1329.
If set to yes, the module cyber_charge() is called, which resides in
order.pm. Here, the data are prepared for the transaction. The actual
transaction is then handled by CCLib.pm.

If you cannot find your problem, simply by following the path, you may
want to write some info at various points into a temporary log file to
see where your code is failing.

Mind you, there may be a much simpler solution. Someone may be able to
hint you more precisely to the cause of your problem, since there must
be quite a number of folks out there who have been able to get Cybercash
running.
