Many many thanks. With just slight modification of you example code I was able
to make it work. Here is my modification.

$new_quantity = 0;
# Loop over all items in the main cart:
foreach my $item ( @{$Safe{'carts'}->{'main'}} ) {
# Set quantity to the value of the variable $new_quantity
# This condition will set only for the current item as defined
