cgi inf ACTION="/cgi-bin/flycat/process" not found

I successfully installed MiniVend (3.14-4) on my server (LINUX 5.2).  I
use Apache secure server.  I installed flycat and can successfully get
to the index page (remotely) as follows:

http://www.objectivesolutions.net/flycat/index.html.

There are two problems:

1.  When I try to submit anything I get "The requested URL
/cgi-bin/flycat/process was not found on this server.".  There is no
file named "process" (or "process.cgi" or "process.pl") anywhere to be
found, so I'm not surprised it isn't being executed.

2.  Also, the README for flycat says: "2.  Run the sample order page and
log on as user "flycat" with password "pass"."

There is no login prompt - it takes me straight to a page (check it out
at the url above) with two purchase options and a "Build you own item"
form.  Is the README wrong?  I did indeed specify "flycat" when I ran
makecat (bin/makecat flycat).

Any explanations of these two issues would be greatly appreciated.
