Take a look at feedback file, in the simple demo.  It contains 2 files, one
is the form, and the other will send mail.  Hope this help.

Hi!
I have several different forms in my web site, apart from the checkout
form.....can i use the mv_email_order for all of the forms? (changing the
necesary fields and stuff), or should i use a cgi formmailer?
