This happened to me several times as I stumbled through my implementation
of Minivnend.  In  every case it was due to a syntax error, missing tag or
similar SNAFU which I had created while editing the source for
checkout.html.  Try switching back to the unmodified checkout.html - there
is a copy in the /mvend/pages/ord path - and see if the problem goes away.
