This happened to me before.  The checkout page contains a javascript which
will check to see if user should be charged sales tax, based upon his home
state vs. the setting of __TAXAREA__, and will advised him as such.  I made
some changes to the checkout page using a WYSIWYG HTML editor and it chewed
up this code and things stopped working.  You should start by loading the
unmodified checkout.html file from your /mvend directory and see if that
solves the problem.  Also, check the value of __TAXAREA__ in catalog config.
