Tyler,

First thing that strikes me is that your price points overlap.

price ranges should not overlap IE:

0	0	e nothing to ship
0	29.99	7.50
30	99.99	10.40
etc...

second,

You should use :
[subtotal noformat=1]
instead of price, I don't know why but for me this worked and price
did not work.
