Well,

I did the following:

touch .access in the config subdirectory
../bin/htpasswd.pl catalog.cfg demo

I still can't get the authentication to work.  I keep getting the
following errors:
nhatrang.hacom.net NJbHPIAd:minivend - [16/July/1999:22:33:53 -0400]
simple /sim ple/config/menu auth error host=nhatrang.hacom.net
ip=209.45.241.10 script=/simple page=/config/menu
