Minivend --> CyberCash 3.2

Hi All,
    Just finished setting up CyberCash on the system
here.  I used their test harness, and it works fine.  When
I go to use CyberCash with MV 3.11, it comes back with an Order Failed
page.

    In checkout.html, I have defined:

name = required
address = required
city = required
state = required
zip = zip
country = required
email = required
phone = required
&fatal = yes
 
&set = mv_payment CyberCash
mv_credit_card_info = required
mv_cyber_mode = yes
&final = yes
&set = mv_email [value email]
[/set]
 
    Any ideas?  I know that the module is loading at the MV daemons start, and I also added the path to the CyberCash config file, and what version of CyberCash I am working with... any ideas?
