Multiple Checkout Pages

I want to add an extra step to the checkout process. I'm having trouble 
getting this to work. In my checkout.html page,

- what should I set the mv_todo, mv_nextpage, mv_successpage, etc. to?
- what do i have to change in the catalog.cfg file?
- what other changes need to be made?

Here's a flow of what i'm trying to do:

Old Method
--------------------------------------------------------
[user shops]

 --->basket.html
[user clicks "check out"]

 --->checkout.html
[user fills out address, credit card info, etc.]
[user clicks "place order"]

--->thankyou.html
[user sees a thank you note]


New Method
--------------------------------------------------------
[user shops]

 --->basket.html
[user clicks "check out"]

 --->checkout.html
[user fills out address, credit card info, etc.]
[user clicks "calculate total"]

--->checkout2.html
[user sees total including tax, shipping, service charges, etc.]
[user clicks "place order"]

--->thankyou.html
[user sees a thank you note]
