There are many reasons why this can happen. Check you error.log files.
Please include more specifics, such as on what page you are selecting
"Place an Order", what the failure page says, how far along you are in
setting up Minivend, and key lines you have set from your minivend.cfg and
catalog.cfg files, messages in error.log files, etc.
