HTML form based interactive SQL query

I would like to make a form-based query to my SQL database.
For example:
how to send the following query to be sent to my database (the
form_field_artist is the user input field on the form):
select code from products where artist like form_field_artist
I have no problems with predefined (hard-coded) queries but no idea how to
make an user input influence at the query results.
In documentation I haven't found any word about this, and if there was
something alike it's not working for me.
If you can help me on this, by sending for example piece of a page I will be
greatfull.
