Credit Card not taking

I am rebuilding this minivend installation and for some reason the 
credit card is never accepted.

The only time a transaction can be pushed through is when the user 
selects 'I will call with my information'.

When I try to use a credit card I get the 'you need to verify your 
credit card info' error message on the Missing Field(s) html page.

I went back to the default catalog checkout.html page and it gives me 
the same problem.
